JOB SUMMARY

This position shall be responsible for providing non-skilled nursing care to an assigned group of patients in the home care setting.

  2. Observes patients and reports observations and concerns to the primary care nurse and/or Patient Care Coordinator, Nurse Facilitator or Manager of Inpatient and Bartholomew.
  3. Completes patient records accurately and on a timely basis.
  4. Assists in maintaining a clean, healthful environment for the patient.
  5. Attends and participates in Interdisciplinary Group meetings, in-services, and staff meetings as requested.
  6. Participates in a minimum of twelve- (12) hours in-service education per year.
  7. Assists in orientation of new Home Health Aides and Homemakers.
  8. Promotes the Hospice benefit and provides education to all potential referring customers including but not limited to case managers, nursing facilities administrators and staff, physicians and physician office staff, and community groups.
  9. Completes other tasks as assigned.

JOB SUMMARY

This position shall be responsible for meeting the nursing care needs of assigned patients/families in community and home-based settings.  Professional nursing services will be given in accordance with physician’s orders and the plan of care established by the Hospice team.

RESPONSIBILITIES

  1. Makes routine Hospice nursing visits as assigned to patients in the home, extended care facilities, assisted living facilities, and hospitals.  Completes assessment of the patient/family needs including physical, emotional, and bereavement status.
  2. Performs as case manager maintaining responsibility for assigned patients in a cost-effective and quality manner.  Manages time, reimbursement of visits, pharmacy and medical supplies, and durable medical equipment.  
  3. Teaches, supervises and counsels' patient/family care techniques including pain and symptom control, dietary instruction, use of equipment and observation techniques.
  4. Develops and implements the plan of care for each patient in collaboration with the patient, family and Interdisciplinary Group (IDG).  Evaluates each patient/family as directed and makes appropriate changes to the plan of care.
  5. Communicates with and updates other IDG team members as needed to coordinate and ensure quality care (e.g. Social Services, Home Health Aides, Volunteers, and Medical Director).
  6. 6. Promotes the Hospice benefit and provides education to all potential referring customers including but not limited to patients and family members, case managers, nursing facilities administrators and staff, physicians and physician office staff, and community groups.
  7. Maintains accurate records in accordance with the supervising organization’s policies and procedures in a timely manner (e.g. initial plan of care, comprehensive plan of care, updates to the plan of care, daily documentation requirement, discharge summaries, death summaries).

QUALIFICATIONS

  1. Education – Bachelor’s Degree in Nursing Science (BSN) or currently enrolled in an accredited program.

    • Licensed Registered Nurse in the State of Indiana
    • Valid Driver’s License and car insurance

JOB SUMMARY

This position shall be responsible for providing counseling and supportive services to seriously ill patients in the home setting that encourages goal setting and the patient / family adjustment to their illness, according to individual and/or family needs.

RESPONSIBILITIES

  1. Provides services as indicated by the Palliative Support Registered Nurse and medical staff.
  2. Assists patient/family in locating simple legal forms (e.g. Living Will, out of hospital DNR, General Power of Attorney, Health Care Representative).
  3. Provides psychosocial assessments and psychological counseling for the following but not limited to:
    • Patient/family adjustment to illness.
    • Individual counseling relating to stress, anxiety, depression, self-esteem and feelings of inadequacy.
    • Family counseling relating to social/emotional problems.
    • Coordination of services for palliative care including environmental, equipment, temporary care services, long term care and community support agencies.
    • Economic counseling to assist in receiving financial aid applications.
    • Grief counseling.
  4. Provides interventions to enhance the quality of life of patients and families through assessment and treatment of pain and other symptoms.
  5. Provides education related to health care choices for the patient and family and advocates effectively for patient comfort, dignity and choice.
  6. Identifies and addresses the need for Advanced Directives including Do Not Resuscitate (DNR), Living Will and Health Care Power of Attorney.
  7. Assists with coordination with necessary community resources.
  8. Participates in Palliative Support Care Team meetings.
  9. Collaborates with the Palliative Support Care Team to improve the quality of life of the patient and family based on the patient’s established goals of care.
  10. Coordination of services with the patient’s referring Physician, Specialists and hospitals as necessary to promote patient and family satisfaction as well as continuity of care across the health system.
  11. Prepares and assists patients and caregivers for transitions from palliative when appropriate.
  12. Documents communication, findings, care provided and outcomes per palliative care standards.
  13. Participates in the data collection process for defined key metrics for continued program development and success.
  14. Promotes palliative care and provides education to all potential referring customers including but not limited to patients and family members, case managers, nursing facilities administrators and staff, physicians and physician office staff, and community groups.

  2. Experience
    • Applicants must have a minimum of two years’ experience in hospice and palliative care.
    • Acute Care and/or Home Health experience preferred.
    • Has three-five years of social work experience in a health care setting.

       

  3. Licensure
    • Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW), Indiana Licensed.
    • Valid Indiana Driver’s License.
  4. Certification- specialty certification preferred

JOB SUMMARY

This position handles all transactions involving patient accounts.  This encompasses billing for services provided, applying payments to accounts, and analysis of accounts receivable for appropriate action.  Responsibilities utilize knowledge of computer systems, office procedures, medical coding, Medicare and Medicaid regulations, working with commercial and managed Medicaid payors, and Our Hospice of South Central Indiana goals and objectives.

RESPONSIBILITIES

  1. Maintains patient financial files, including information on insurance coverage, amounts invoiced, payments received, adjustments made.
  2. Verifies insurance benefit coverages for patients upon referral, and afterwards as needed.
  3. Coordinates submission of pre-certification, pre-authorization, and prior approval documentation on a timely basis.
  4. Communicates and collaborates with other departments to obtain required information for billing purposes.
  5. Applies payments to appropriate patient accounts.
  6. Adjusts patient accounts as directed by the Director of Business Operations to reflect write-offs and bad debt expense.
  7. Follows up with payor sources to ensure timely receipt of payments.
  8. Ensures Change of Status and Level of Care information is entered properly for billing purposes.
  9. Identifies issues with billing software and works with Hospice IT to obtain resolution in a timely manner.
  10. Prepares monthly reports of amounts billed and adjustments made.
  11. Review Medicare, Medicaid and commercial insurance company bulletins to ensure billing practices conform to stated requirements.
  12. Cash and checks deposits for Hospice, Palliative, and any fund raising events.
  13. Certification of Terminal illness (CTI) and Recertification tracking reports.
  14. Upload medications every week for Medicare billing.
  15. Reviews and ensures medical coding for outgoing claims is consistent with applicable payor regulations and facilitates reimbursement.
  16. Works with patient/families to complete inpatient financial assessment forms.
  17. Runs physician visit reports to review activity and coding for accuracy and completeness.
  18. Runs and reviews billing audit report to identify issues prior to claims processing.
  19. Billing Medicare and Medicaid claims, follow up on timely paperwork for precertification, calling on Medicare and Medicaid claim status, follow-up Medicare claim in the DDE program.
  20. Audit UB-04 Forms for compliance with appropriate Medicare and Medicaid regulations.

  2. Experience – Two years experience in medical accounts receivable or equivalent combination of education and experience
  3. Licensure – Valid Indiana Driver’s License.
  4. Certifications – Certified Medical Coder preferred
  5. Hospice Billing experience preferred
  6. Skills
    1. Understand of financial and reimbursement terminology and concepts.
    2. Must have working knowledge in MS Word, Excel, and the Internet.
    3. Excellent verbal and written communication skills required. Ability to add, subtract, multiply and divide in all units of measure using whole numbers, fractions and decimals.

JOB SUMMARY

This position shall be responsible for explaining the Hospice program to referrals, assessing the patient/family status, advise on appropriateness of admission based on the Hospice admission criteria, and will perform admissions.  Will seek education and learning experiences to maintain current knowledge and skills.  Performs responsibilities utilizing knowledge of office systems and processes, regulations and Hospice goals and objectives. 

RESPONSIBILITIES

Essential

1.      Makes the initial contact on referrals as required, explaining Hospice services and evaluating the patient’s ability to meet Hospice’s admission criteria.

2.      Responds timely to all referrals, fostering relationships of trust, responsiveness, and open communication with all referring customers including but not limited to hospital case managers, physicians, patients and families.

3.      Promotes the Hospice benefit and provides education to all potential referring customers including but not limited to case managers, physicians and physician office staff, and community groups.

4.      Assists in the evaluation and transfer of the patient to the Inpatient Facility and other contracted facilities as indicated.

5.      Makes visits to hospitalized patients for the purpose of evaluating education, providing patient / family support and assisting in the discharge planning process as necessary.

6.      Performs as admission nurse in the home, extended care facilities, and assisted living facilities.  Completes admission and begins assessment of patient / family needs including physical, emotional, and bereavement status.

7.      Insures optimal communication and reimbursement by providing for documentation of services rendered that is compliant with state and federal regulations.

8.      Participates in on-call service to patient/family to insure availability on a 24 hour, seven day a week basis.

9.      Completes and documents all admission paperwork using point of care service in a concise manner with the patient/family, targeting meeting the patient goals of care.

10.  Voicemails appropriate teams and or departments only pertinent information.

11.  Collaborates with Director or designee to assure completion of duties, scheduling and arranging coverage for referrals and admissions.

12.  Collaborates with Referral Supervisor and hospice MD/NP regarding medical eligibility and paint the picture documentation.

13.  Exemplifies time management skills to complete at least 2 admissions in a work day.

14.  Builds collaborative relationships with Referral and Admission Team members, Interdisciplinary Teams, Referral Coordinators, and others to assure coordination and continuity of care and services.

16.  Communicates with all community partners and referral sources, as well as with families in a positive manner, consistently seeking win-win solutions.

17.  Meets with patient/family to identify needs and offer hospice solutions.

18.  Reviews relevant medical records, assessing for documentation for support hospice medical eligibility and goals of care.

19.  Evaluates coverage/relatedness of care/treatment/services based on the hospice patient care and treatment coverage guidelines.  Informs others for all care/treatment/services needed at admission.

20.  Begins the interdisciplinary assessment based on immediate patient/family needs, as outlined in the plan of care established for patient admission.

21.  Identifies immediate education needs of the patient/family and addresses those needs.

22.  Evaluates patient/family outcomes and satisfaction with care/services provided.

23.  As appropriate during admission, may begin the process for preparing family/caregiver for the dying process, including signs and symptoms of impending death and notification of hospice at the time of death.

24.  As appropriate during the admission process, meet with varying facility care staff.  Assess staff for hospice/palliative care education needs, communicate plans for and build a collaborative relationship with external customer. 

25.  Completes requests for other services (Certified Nursing Assistant, Volunteer, other) and initiates plan of care when service need is identified upon admission.

26.  Completes the admission Plan of Care, in coordination with the patient/family. 

27.  Demonstrates the ability to work autonomously, with efficiency, time management and strong work ethic to see the task to completion.

28.   Participates on assigned committees and/or staff meetings (e.g. Tumor board, Cancer Care Committee, Palliative Care Committee).

29.  Participates in the provision of educational and community relations activities to the community and other health care professionals as requested. 

30.  Completes medication reconciliation at visits comparing profile to pharmacy profile and medications present in the home or facility.  This includes notifying the pharmacy of changes in orders.

31.  Provides support to the Hospice Inpatient Facility as needed in the accomplishment of effective patient / family care.

32.  Consults with appropriate Patient Care Manager/Director Clinical Services for questions and concerns regarding urgent needs/issues that arise.

33.  Demonstrates competency in understanding quality metrics and is accountable for following standards and best practices in order to contribute to organizational excellence.

Additional Duties & Responsibilities

1.      Performs additional duties and/or responsibilities as delegated by the Director of Clinical Services including but not limited to direct patient care, after-hours visits (i.e. symptom management, death visit, post fall visit), admissions, discharges, preparing family and caregiver for dying process, medication administration and treatments.

2. Provides assistance with medical record workflow including but not limited to audits, physician order approvals and patient transfers within the electronic health record.

QUALIFICATIONS

1.      Education – Bachelor’s of Science in Nursing (BSN); or currently enrolled in an accredited program. 

2.      Experience – Preferred: One to three years’ experience in health care.

3.      Licensure

a.      Licensed Registered Nurse in the State of Indiana.

b.      Valid Driver’s License and car insurance.

Summary

Our Hospice of South Central Indiana is seeking a Hospice & Palliative Care Physician for their Columbus, Indiana facility to serve as Medical Director.

The Hospice Physician will provide direct patient care and symptom management to hospice patients while assuring care is in compliance with Medicare and other regulatory programs.  The clinician will also serve as liaison between the primary physician and the hospice team, providing care and facilitating communication and continuity of the care plan.  The clinician will also support the admission and care delivery process of hospice patients.  

Primary Responsibilities include:

  1. Responsible for the medical component of the hospice’s patient care program.
    • In conjunction with the Interdisciplinary Group (IDG) is responsible for or participates in making decisions regarding admissions to hospice home care or inpatient settings(s).
    • Participates in the development of individual plans of care before admission of a patient/family to the program
    • Acts as a liaison between the attending physician and the IDG.
    • Acts as primary/attending physician when patients have no primary/attending physician.
    • Participates in the development, evaluation, and revision of written care plans by attending the IDG meetings.
    • Participates in medication review, titration, or discontinuation, in line with patient’s condition and goals.
    • Participates in IDG, completing medication review, relatedness and clinical decision making.
    • Is available to have goals of care conferences with patients and families who are considering the choice of hospice, to help counsel and coordinate care in line with patient’s condition and goals. These visits can be required either:

                   - Before choosing hospice (such as a palliative care consult).

                   - While under the care of hospice. 

  2. Works collaborative with other medical staff.
    • May be asked to assist with work assigned to other medical colleagues as work load indicates.
    • Rotates for weekend and holiday call to provide for the 24 hour, 7 day a week needs for hospice in all levels of care.
  3. Visits hospice patients as indicated.
    • Makes visits to hospice patients whether at patient’s home or a residential facility as indicated for symptom management or re-certification of hospice eligibility.
    • Makes visits to hospice patients in the Hospice Inpatient Facility as indicated.
  4. Completes all required documentation in a timely manner.
    • Documents patient visits per State and Federal regulation.
    • Completes death certificates, CTIs and other documentation as needed.
  5. Responsible for making quality, cost effective medical care decisions.
  6. Serves as a liaison with community physicians, medical schools, hospitals and nursing homes.
  7. Advises Medical Director of exceptional conditions, such as unanticipated adverse outcomes in the clinical setting.
  8. Stays up to date on notifications, emails and Chex educations, and is responsible for updated information.
  9. During work hours, is available to discuss treatment options, make recommendations for symptom management in patients who are considering or enrolled in hospice
  10. May involve direct discussion with referring MD to clarify needs and expectations.
  11. Will fulfill the role of Medical Director as Medical Director Designee, when Medical Director is unavailable for protracted times.

QUALIFICATIONS

  1. Education – Doctor of medicine or doctor of osteopathy degree.
  2. Experience
    • At least two years’ experience in a medical specialty dealing with terminally ill patients, such as oncology or geriatrics and with demonstrated experience in the principles of pain and symptom management.Hospice and Palliative Medicine board certification preferred.
    • Five to ten years’ experience in health care administration with good managerial skills; strong oral and written communication skills; good interpersonal and motivational skills.

  3. Licensure
    • Licensed Physician in the state of Indiana.
    • Board certification by an American Board of Medical Specialties approved board examination, an osteopathic medicine equivalent.
    • Board certification in Hospice and Palliative Medicine, preferred
    • Must possess a valid Indiana driver’s license, and current auto insurance.
    • Hold a current and updated DEA license
    • Obtain and maintain privileges at Columbus Regional Hospital.
    • Remain current in CME hours in a primary specialty.

JOB SUMMARY

This position provides administrative and clerical assistance to the Inpatient Facility of Our Hospice of South Central Indiana every other weekend.  This position performs responsibilities utilizing knowledge of office systems and procedures, regulations and Hospice goals and objectives; and contributes to the development and/or modification of operating processes. 

RESPONSIBILITIES

Essential

  1. Answers and screens all incoming calls on a multiple line system.Directs calls appropriately to internal staff and pages external staff (RN’s, HHAs, Chaplains, MSWs, etc.) as needed.Receives all walk-ins in a courteous and helpful manner assisting them and directing them to appropriate individuals(s) or office(s) of Hospice.
  2. Daily enters and updates information in Our Hospice of South Central Indiana’s software systems, e.g. relevant patient information.
  3. Responsible for EMR workflow, electronic consent workflow, scanning documents and input as needed daily.
  4. Ensures that documents requiring physician signature are tracked for completion and followed up appropriately.
  5. Order DME for patients in the IPF or when the IPF patient is transferring to the home setting.
  6. Ensures the on-going accuracy of active charts and verifies accuracy of closed charts.
  7. Creates Interdisciplinary Group (IDG) discussion list, agenda and console.After IDG meetings, copies, fax, mail and files IDG information.
  8. Maintains integrity of logs kept within the Inpatient Facility.
  9. Works independently and within a team on special and nonrecurring and ongoing projects, including fundraising events.Acts as a project manager for special projects at the request of the Executive Assistant, which may include: planning and coordinating multiple presentations, disseminating information and organizing office events.
  10. Coordinates meetings, gathers data, prepares correspondence, documents and reports, maintains files and records on behalf of the Executive Assistant and Manager of Inpatient Facility as needed.
  11. Prepares and distributes daily Inpatient Facility census reports for the organization.
  12. Scheduling assignments for IPF RNs, HHAs and MDs.Processes Bereavement cards for patients that have passed.
  13. Places weekly linen orders.
  14. Demonstrates ability to remain calm and functional in work situation and follows directions and established emergency protocols, i.e. Fire Drills.

  1. Performs other duties as needed with regard to special projects.
  2. Performs all other general office duties as assigned promptly and accurately, including typing, copying, filing, and faxing documents, preparing reports/documents for both internal and external customers, and maintaining logs.

This position shall be responsible for coordinating the referral to admission process including: managing the intake for referrals, explaining the Hospice program, collecting and reviewing documentation for eligibility of the Hospice admission criteria, coordinating with the clinical team and scheduling admissions. RESPONSIBILITIES

  • Makes the initial contact on referrals as required, explaining Hospice services and assists the Business Development Representatives in evaluating the patient’s ability to meet Hospice’s admission criteria.

  • Manages daily and weekly activity reporting to business development team, clinical team and leadership to promote communication and teamwork.
  • Manages accurate and timely input of new referrals into the system.
  • Collaborates with the TCM to coordinate the admission schedule and availability. Coordinates the transfer of the patient to contracted facilities or home as indicated.
  • Facilitates communication and works effectively between Hospice staff, patients/families/caregivers, physicians, vendors, and other professionals as appropriate.
