How To Apply

Please submit all required materials below via the online PhORCAS system by the deadline set by PhORCAS.
- Current Curriculum vitae
- Letter of intent
- Three letters of recommendation via PhORCAS
  - One must be from a clinical preceptor
  - One must be from an employer
- Official college transcript(s)
All applicants must register for the Resident Matching Program via the ASHP Match website at www.natmatch.com/ashprmp
Upon evaluation of application materials, qualified applicants will be offered an on-site visit and interview with the residency team or equivalent as determined by the program. On-site options may not be available at this time. Please be sure to contact the Residency Director for more information regarding the interview process.
Interviews take place in late January and February. Notification of interview will occur no later than January 31^st.
Successful candidates will be expected to graduate with a Doctor of Pharmacy degree and obtain Indiana pharmacist licensure by August 1^st.
The application process will include a formal employment application to be completed upon successful matching with our program.
