Registration

Our goal is to make your visit as streamlined and convenient as possible. Upon your provider’s referral to our service, our team will contact you by phone to begin your registration process, schedule your first appointment and answer any questions you may have prior to your first visit.

Cost & Payment

In most cases this service will be covered by your insurance provider with a co-pay. This co-pay amount will be billed to you at the end of each month. We will work with each patient to ensure cost does not limit access to the service with help from patient financial services.

Parking

The Medication Management Clinic is located on the first floor of Columbus Regional Hospital next to the Emergency Department. Please use Entrance 1/Emergency Department Entrance to visit the clinic.

Our goal at the Medication Management Clinic of Columbus Regional Health is to partner with you to provide the most complete and thorough care when it comes to using your medications. By providing customized education we can help you get the most benefit from your medications while minimizing side effects and complications.

Anticoagulation Services

Patients with a variety of health conditions require the use of medicines that help prevent excess clotting of the blood. This group of medicines is commonly referred to as anticoagulants. Coumadin™ (warfarin) is one such medicine that needs routine monitoring by your health care team to make sure your blood is thinned appropriately without experiencing unwanted side effects.

Anticoagulation monitoring by the Medication Management Clinic pharmacists will consist of routine blood testing and review of the many factors that can affect how much Coumadin™ (warfarin) each individual needs.

What to Expect

Your partnership with the Medication Management Clinic will begin with a call from our staff to schedule your first appointment.

Your first appointment will last about 45 minutes to allow us time to get to know you better and provide personalized medication education. We will introduce or review the use of Coumadin™ (warfarin) with you and discuss how we will work with you and your doctor to ensure the safe use of this medication.

After your first appointment, your following visits will only last about 15 minutes as we evaluate your current routine, provide education, and make adjustments as needed.

  • Upon arrival to the clinic, a simple finger stick blood sample will be taken to help us evaluate how well your medicine is working to prevent blood clots.
  • Your pharmacist will review these results as well as any changes in your current medications and general health to decide if changes should be made to provide the best therapy for you.
  • Personalized education will be provided to help you use Coumadin™ (warfarin) safely and effectively.
  • If a different strength of Coumadin™ (warfarin) is required, your pharmacist will arrange to have your prescription filled by your preferred pharmacy.
  • Prior to leaving the clinic, your pharmacist will provide you with clear instructions on how to use your Coumadin™ (warfarin) and will schedule your next visit to the clinic.
  • At the completion of each visit to our clinic, your doctor will receive a complete report sharing what was discussed and changes that were made to your Coumadin™ (warfarin) therapy.
