Appointment Scheduling

To schedule an appointment at Sandcrest Family Medicine, please call our office at 812-373-2700. We are accepting new patients, new OB patients and newborns at this time. Please view individual physician profiles on our website to confirm each doctor's accepting status.

When you call for an appointment, the scheduler will inquire as to the nature of your needs.  The information you provide will enable us to schedule the appropriate amount of time with your provider.  Please try to select one of our providers as your preferred choice and schedule all appointments with this provider.  Other providers offer acute services for you if your primary provider is unavailable.  Please arrive 15 minutes prior to your appointment so that we may have time to check your information and be ready to see the provider at the appointment time.  Please provide your insurance card at each visit and inform us of any changes in name, address or telephone number.

If you cannot keep your appointment please call during regular office to cancel.  The courtesy of canceling an appointment will allow us to use this time to care for other patients.

We require no smoking, no perfumes and no cell phone usage in the office.
